To fully understand your unique case and best serve you, we collect information on your medical history and conduct a thorough physical examination. Once we complete the initial consultation, we will compile these documents into a comprehensive file and create a customized treatment plan to meet the patient’s goals. This also allows us to communicate and work with the patient’s other health providers, including any specialists, to provide patients with the highest chance of success. We can modify and alter treatment plans as necessary throughout your time with us. We encourage patients to communicate any new health changes or symptoms they experience immediately.

    New Patients

    New patient documentation allows our team to develop a personalized treatment plan for your unique needs and preferences. You can fill out this information in our "New Patient Information Form" before your appointment. You will want to have all your insurance information ready to bring with you, along with a list of your prescriptions and current medical conditions. Be sure to include any over-the-counter medication and herbal supplements you take as well.

    If you do not feel comfortable filling out the form online, we can help you with it on the day of your appointment. Please bring the relevant information with you.
